Three New Printable Forms on the DSP Handbook App!
You asked and we delivered! In response to your requests, we’ve added three new printable forms to the DSP Handbook app.
Blood Pressure Log:
Sometimes a person may have high blood pressure, otherwise known as Hypertension. The treating medical professional may want to obtain a record of the person’s blood pressure prior to prescribing a medication or may want a record of the blood pressures while the person is taking the medication to make sure it is working properly. You can use this log to record the blood pressure and heart rate (pulse) readings you take at home. Write the date and time of day you take them. Write the blood pressure with the systolic number on the left and diastolic number on the right: For example, 120/80.
Blood Sugar Log:
If the person you support is diabetic the treating physician may want a record of the person’s blood sugar levels. People who take an injection of insulin will need to have their blood sugar recorded at least once per day but most will have it taken multiple times. Sometimes a blood sugar reading will also need to be recorded if the person is on oral medications only. Use this log to record all blood sugar readings to report to the medical professional. Include any notes that may explain why a blood sugar reading is higher or lower than expected, such as illness or exercise.
Bowel Movement Log:
Many people with intellectual disabilities are on medications to prevent or relieve constipation. Using a bowel management chart is an easy way to communicate if they have had a bowel movement and if it is unusual and requires treatment. This log is designed to help doctors measure the time it takes for food to pass through your body and leave as waste. The shape and form of the stool may also point the doctor toward a diagnosis of some digestive problems.

This information is collected to help us to have some understanding of our audience so we can provide useful, relatable content for you. Information is only used internally and never shared with any third party.
